ABOUT US Vital Health Records is a community-based program. Its aim is to provide health forms for each family member to fill in their vital health information (such as medications, medical conditions, etc.).

Those vital health records are placed in the family's refrigerator. Then, if emergency medical services are called, they will know where to find the forms immediately after they arrive.

By having current vital health records available, emergency crews will save valuable time in responding to your emergency.
